Cancellations, Refunds, and Chargebacks
A full session fee is charged for missed appointments or cancellations with less than 24-hour notice. Client will be expected to pay cash or check at the end of each session. A copy of client's credit card will be kept on file and Jennifer Shapiro-Lee, LCSW shall be authorized to charge Client’s Credit Card for any unpaid invoices. Client shall not make any chargebacks to Company’s account or cancel the credit card that is provided as security without prior written consent. Client is responsible for any fees associated with recouping payment on chargebacks and any collection fees associated therewith.
Controlling Law & Jurisdiction
This Agreement is governed by and shall be construed in accordance with the laws of the state of New York, and in the event of any dispute arising in relation to this Agreement or any dispute arising in relation to the Site whether in contract or tort or otherwise the state of New York courts will have exclusive jurisdiction over the dispute, unless mandatory applicable laws require otherwise.
